Enrollment is open for Adolescent Health Programs

FCDPH's Adolescent Health Programs are officially accepting applications for the 2024-2025 school year. Flourish, Motivating Males on the Move (M3), and Unify will each be accepting 25 participants each and each program will run from September 2024-June 2025. Along with health education, all programs provide free transportation, snacks, activities, games, and field trips to participants throughout the year. If you are interested in receiving more information or applying, please fill out the referral form using the link or QR Code in the flyer.
