The Forsyth County Juvenile Crime Prevention Council announces the availability of up to $713,108 in intervention and prevention funds for services to youth most at-risk of court involvement. JCPC funds require a local cash or in-kind match of 30%. Based on the local JCPC planning process conducted to date, the continuum of program services and structures needed in Forsyth County have been identified, in rank order, as the following:
After submitting the application electronically, print and submit hard copies (one original and 11 copies that are 3-hole punched and paper clipped) to the local office. Private non-profits are also required to submit No Over Due Tax forms, Conflict of Interest Statements (DJJDP 13 001), and proof of 501(c)(3) status. One (1) copy of previous fiscal year’s audit and a current list of the Board of Director’s complete with contact information are also required. Continuation programs must submit their 2010-2011 measurable objectives and the first 6 months of 2011-2012.
