Do you have a health care power of attorney assigned? Do you have a living will? Join us on Monday, Nov. 13 at 1 p.m. at the Lewisville Branch Library to learn about the purpose and importance of having advance care planning in place. Advance care planning is a process to help your loved ones know how to speak for you if you are ever unable to make your own medical decisions. Such instances can include if you undergo surgery, get a serious illness, or are injured in an unexpected accident.
A Trellis Supportive Care representative will discuss these topics and provide the necessary documents for you to assign a health care power of attorney and create a living will. They will also provide guidance for filling out these forms at this program. Registration is encouraged though not required. Register by calling the Lewisville Branch at 336-703-2940.
