Adult Medicaid provides healthcare coverage for qualified adults who are 65 or older, blind or disabled (permanently or totally disabled for at least twelve months).
Medicaid programs range from full to partial benefits based on income, age, health needs, resources, and other eligibility requirements. Medicaid may help pay for the costs of nursing homes,
intermediate care, and long-term care in the home. Additionally, eligible Medicaid beneficiaries may also receive cash payments from the Special Assistance program.

The Breast and Cervical Cancer Screening program provide breast and cervical exams, Pap smears, HPV testing, and mammograms for women ages 40-64 who meet specific income guidelines.

The Cleveland Avenue Dental Center is part of the Forsyth County Department of Health and Human Services. We serve children and adults of Forsyth County and surrounding areas. We accept Medicaid, NC Health Choice, Ameritas and self-pay patients. A sliding fee scale may be applied for self-pay patients based on household income. We are a fully functioning dental center for both children and adults. Our caring, compassionate staff will help ensure that you have a pleasant dental experience.

Control of communicable disease in Forsyth County is handled by a team of Public Health nurses. Our main goal is to keep the public safe by preventing the spread of communicable disease. Reports are received from physician’s offices, labs, schools, daycares, food establishments and long term care facilities. Concerned citizens also call in with complaints. The Communicable Disease Nurse and Environmental Health Specialist may do an investigation jointly depending on the disease and the area involved within the community.
The NC Division of Public Health/Communicable Disease Branch has revised surveillance forms and supporting documents. The Confidential Report of Communicable Disease Report Form - Part 1 is used by health care providers and local health department staff to report and investigate diseases that represent a public health threat according to 10A NCAC 41A.0101 Reportable Diseases and Conditions.

Forsyth Regional Opioid and Substance Use Team is made up of representatives from over 80 different community agencies in addition to community members with lived experiences. We have a comprehensive plan and are working together to build and maintain a continuum of care in the Forsyth County community that educates the community, provides primary prevention, overdose prevention, promotes harm reduction practices, removes barriers to treatment and recovery, and breaks down stigma surrounding substance use.

The Men's Health Program is a male based community outreach program that helps men become knowledgeable of chronic diseases and prevention. This program focuses on chronic illness and other health topics such as nutrition, physical activity and mental health.

MIH supports individuals who are facing substance use or mental health crises by providing specialized care and connecting them to appropriate resources. We focus on improving community health through advanced, trauma-informed care.
Services include follow-up support after overdoses and help with finding treatment options like medication-assisted therapy. We also connect people in crisis with mental health resources to meet their needs and improve their quality of life.
Our team is made up of paramedics and peer support specialists who guide individuals through the recovery process and help them find the resources they need. We’re here to assist anyone facing a mental health crisis or struggling with substance use.

The POSSE Team provides HIV and STI screenings and education in Forsyth County community locations, such as churches, jails, universities, homeless shelters, nightclubs, and community fairs.

The Refugee Program helps improve refugees' health after arriving in the United States by providing health screening and vaccinations. I-693 services are also available.

The STI program includes confidential examinations, diagnostic testing and treatment, a physical exam, appropriate labs, and counseling for syphilis, gonorrhea, chlamydia, HIV, and other sexually transmitted infections.

The Forsyth County Stepping Up Initiative houses two programs: Stepping Up Process to End Recidivism (SUPER) and the Mental Health Court (MHC).
Tuberculosis Control and TB Skin Tests are NOT availble at this time.
Children require regular checkups throughout childhood to monitor health, growth, and development. Forsyth County Well Child Program offer appointments for children from birth through 21 years.
